• Clinical Nutrition (Supervised Practice) Program Application

• A $15.00 Application Fee

• Offical Transcript(s)(from each college/university previously attended. (unofficial transcripts are acceptable for initial review.)

• Copy of Official Graduate record exam Score (unofficial report is acceptable for initial review)

• Three ADA Recommendations

• Personal Letter of Application

• Declaration of Intent to Complete Degree or Verification Statement obtained from your ADA Approved Didactic Program in Dietetics

• Appointment Day Information

• Entry into ADA Dietetic Internship Match
