Preceptors will generally receive evaluations and guidelines when an intern arrives for a rotation. However, there are times when those are misplaced or not received. When that happens, evaluations can be printed from this site. For general guidance, we have included files of general instructions and definitions. For each type of evaluation, we have also included the forms and guidelines that will assist you in completing specific evaluations. The professional attributes evaluation should be used in every rotation. The Nutrition Care Process Evaluation should cover all management rotations.
