Please follow directions carefully; complete and accurate information is essential. The degree application form and graduation information sheet must be filled out completely and accurately (include cap and gown measurements ). The name you use on the degree application form will also appear on your diploma. This name must be the same name that’s on your University of Memphis record. Please go to Room 003 of Wilder Tower to change your name. - The deadline dates for submitting the intent-to-graduate application is listed below:
- November 14, 2007 - for May 2008 graduation
- April 2, 2008 - for August 2008 graduation
- July 16, 2008 - for December 2008 graduation
- November 12, 2008 - for May 2009 graduation
- Please print this information from the website and submit it to Rm. 202-Ball Hall building. The intent-to-graduate forms must include the following:
- degree application form
- A copy of your degree sheet from advisor (For ITGS-Non-Licensure, Sports Management, Leisure Management, Dietetics and Health and Human Performance majors)
- graduation information sheet
- After submitting your Intent-to-Graduate application form to Room 202 - Education Building, you will make an appointment to see Ms. Mary Lanier at a later date.
- FOR TEACHER LICENSURE STUDENT ONLY: You must have taken and passed the appropriate Praxis II Content Knowledge exam prior to student teaching. Therefore, do not submit the intent-to-graduate application until you have done so.
If you change your name, address, or graduation date, please notify Mary Lanier at 678-4309. It is important that these records be kept up-to-date. AGAIN, CONGRATULATIONS!
